You Are Where You Eat - Experts Say Location Can Influence Overweight, Obesity Rates

For years, people who have been working to achieve and maintain a healthy weight have been told that they need to watch what they eat. As it turns out, it may be just as important to pay attention to where they eat – and where they live, work, and go to school.

Several studies have noted a relationship between location and weight problems, with “culprits” such as urban sprawl, impoverished neighborhoods, and the proximity of schools and fast food joints being blamed for increasing obesity rates.

Several researchers have explored the relationship between the rise of the suburbs and a subsequent increase in overweight and obesity rates. For example, a study conducted by the University of British Columbia and the U.S. Centers for Disease Control and Prevention (CDC) found that residents of the suburbs get less exercise than do their city-dwelling counterparts, because those who live in the ‘burbs have to drive to stores, restaurants, and other destinations, while those in the city can walk.

The researchers who reached this conclusion did so after surveying almost 11,000 residents in and around Atlanta, Georgia. According to an Aug. 3, 2004 article by U.S. News & World Report writer Helen Fields, those who were surveyed were questioned about their income, how much time they spend driving and walking, and their height and weight (in order to calculate body mass index).

Among the findings, Fields reported, was that a person’s likelihood of being obese increased by 6 percent for every hour the person spent in their car every day – an experience that is more common to suburban residents than to those who live in large cities.

On June 12, 2005, USN&WR’s Amanda Spake provided additional data to support the findings Fields had presented:

Since 1960, the number of people commuting to work out of the county they live in jumped by 200 percent. Residential "sprawl" has meant a 250 percent increase in vehicle miles traveled. The average driver spends 443 hours yearly behind the wheel, the equivalent of 11 workweeks.

Driving displaces other activities, like exercise. "Being in a car doesn't do anything for you in terms of being thin," says Lawrence Frank of the University of British Columbia. …

Living in the suburbs is [also] linked to eating more higher-calorie fast food. At least 1 in 4 adults now eats fast food on any given day. For children, 1 of every 3 meals is fast food.

Of course, revelations about the relatively sedentary nature of suburban life don’t mean that city-dwellers are necessarily immune from environmental weight challenges.

In a Sept. 10, 2004 article on the website of the Center for the Advancement of Health, writer Ann Quigley described a study in which poor neighborhoods were found to have a significantly higher number of fast food restaurants that more economically advantaged areas:

“More convenient access likely leads to the increased consumption of fast food in these populations,” says study author Jason Block, M.D., M.P.H., of Brigham and Women’s Hospital in Boston.

Block and colleagues used computer software to map out and analyze the placement of New Orleans fast food restaurants such as Church’s Chicken, Pizza Hut, Subway, Burger King, and Taco Bell. They found that predominantly black neighborhoods had 2.4 fast food restaurants per square mile, while white neighborhoods only had 1.5 restaurants per square mile. …

Higher income and white neighborhoods have more supermarkets than poor and black neighborhoods, which tend to be served by smaller grocery or convenience-type stores. Research shows that supermarkets offer more healthy foods than grocery and convenience stores, according to the study.

Tianna Gaines, an obese 28-year-old woman who lives on public assistance in an economically disadvantaged area of Philadelphia, echoed this conclusion in a May 6, 2008 article in the Philadelphia Inquirer.

In addition to noting that healthy food is more expensive and harder to find where she lives, Gaines told Inquirer reporter Alfred Lubrano that exercise opportunities are more difficult to come by. “I don’t have the money for Bally’s fitness clubs,” Gaines said. “And I can’t run here. They shoot you.”

With dangerously high childhood obesity rates considered by many experts to be one of the modern world’s most pressing public health issues, increased attention is being paid to what, when, and where young people are eating.

Schools have been working to improve the quality of food that is available to students in the cafeteria and elsewhere on campus, but a new study has found that when it comes to schools and healthy eating, the old real estate cliché is appropriate: It’s all about location, location, location.

“Students at schools near fast-food restaurants had higher rates of obesity after controlling for a lot of different variables than students whose schools were not near fast-food restaurants,” Brennan Davis, Ph.D., of Azusa Pacific University, said in a Dec. 23, 2008 article by MedPage Today staff writer Kristina Fiore.

In a study that appeared on the website of the American Journal of Public Health, Davis and his colleagues reported that students whose schools are less than a half-mile from a fast food restaurant are at increased risk for being overweight or obese.

A Jan. 2, 2009 Reuters Health article provided the following details on the research conducted by Davis and his team:

·        The researchers used detailed 2002-2005 data on more than 500,000 middle- and high-school students from the California Healthy Kids Survey to examine whether proximity to fast food restaurants was related to eating habits or body weight.

·        About 28 percent of the study participants were overweight, and 12 percent were obese.

·        Fifty-five percent of the overweight or obese students who were surveyed attended schools within a half-mile of a fast food restaurant.

·        Students who attended schools located near a fast food establishment were heavier than were other students of similar age, ethnicity, and activity level.

·        Students who attend school near a fast food restaurant were less likely to report eating vegetables or fruit or drinking juice the day before, and were more likely to report having consumed soda within the previous 24 hours.

Regardless of where a person lives, works, or goes to school, avoiding overweight and obesity is, in most cases, a matter of eating a healthy, nutritious diet and getting an adequate amount of regular exercise. Economically disadvantaged individuals can face significant challenges, but it is possible to eat healthy on a budget, and following an inexpensive exercise plan can be just as effective as purchasing a costly gym membership.

In the end, even in cases of environmental disadvantage, living a healthy lifestyle is as much a matter of mindset as anything else.
